Job Summary:

We are seeking a dedicated and compassionate Home Health Nurse to deliver high-quality nursing care to patients in their homes. The ideal candidate will have a strong background in nursing, particularly within the pharmaceutical sector, and will be responsible for coordinating and providing patient-centered care that promotes health, wellness, and recovery. This role requires a proactive approach and the ability to work independently while maintaining effective communication with patients, families, and healthcare teams.

Key Responsibilities:

- Conduct comprehensive assessments of patients' health status and develop personalized care plans in collaboration with the healthcare team.
- Administer medications and treatments as prescribed, ensuring adherence to pharmaceutical protocols and safety guidelines.
- Monitor patients' progress and provide ongoing education to patients and their families regarding medication management, disease processes, and lifestyle modifications.
- Collaborate with physicians, pharmacists, and other healthcare professionals to ensure optimal patient care and medication management.
- Maintain accurate and up-to-date patient records, including assessments, interventions, and outcomes, in compliance with regulatory requirements.
- Provide wound care, IV therapy, and other specialized nursing services as needed.
- Educate patients and caregivers on proper medication usage, potential side effects, and the importance of adherence to prescribed regimens.
- Respond to patient inquiries and concerns promptly, ensuring a high level of patient satisfaction and engagement.
- Participate in quality improvement initiatives and contribute to the development of best practices in home health nursing.
- Stay informed about the latest developments in pharmaceuticals and home healthcare practices to provide the best possible care.

Qualifications:

- Bachelor’s degree in Nursing from an accredited institution.
- Valid nursing license in the UAE.
- Minimum of 2 to 5 years of experience in home health nursing or a related field, with a focus on pharmaceutical care.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills to foster positive patient relationships.
- Ability to work independently and as part of a multidisciplinary team.
- Proficient in using electronic health records and other healthcare technologies.

Preferred Qualifications:

- Experience in patient education and counseling, particularly in relation to medication management.
- Certification in Home Health Nursing or a related specialty is a plus.
- Knowledge of local healthcare regulations and standards.
