We are seeking a dedicated and experienced Hospital Case Manager to join our team in Abu Dhabi. The Hospital Case Manager will be responsible for coordinating patient care and ensuring that all medical and non-medical needs are met during their hospital stay. This position requires a high level of compassion, excellent organizational skills, and the ability to work collaboratively with a multidisciplinary team.

Responsibilities:
- Conduct initial assessments of patients to determine their needs and develop a comprehensive care plan
- Coordinate with physicians, nurses, social workers, and other healthcare professionals to ensure the patient's care plan is being followed
- Monitor the patient's progress and adjust the care plan as needed
- Advocate for the patient and ensure that they receive the appropriate services and resources
- Communicate with the patient and their family members to provide education and support
- Coordinate discharge planning and ensure a smooth transition to post-hospital care
- Maintain accurate and thorough documentation of all patient interactions and care plans

Requirements:
- Bachelor's degree in Nursing, Social Work, or a related field

- Knowledge of healthcare regulations and policies in the UAE
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational skills and attention to detail
