We are seeking a highly skilled and experienced Hospital Case Manager to join our healthcare team in Abu Dhabi. The Hospital Case Manager will be responsible for coordinating and managing the care of patients within the hospital setting, ensuring that they receive the appropriate level of care and support throughout their stay.

Key Responsibilities:
- Collaborate with healthcare providers, patients, and their families to develop and implement individualized care plans
- Conduct assessments to determine the needs and goals of each patient
- Coordinate with medical staff to ensure that patients receive timely and appropriate treatment
- Monitor and evaluate the progress of patients, adjusting care plans as needed
- Provide support and guidance to patients and their families, addressing any concerns or questions they may have
- Advocate for patients to ensure they receive the highest quality of care
- Coordinate discharge planning and follow-up care for patients upon their release from the hospital
- Maintain accurate and up-to-date records of patient care and progress
- Stay current on best practices and trends in case management and healthcare delivery

Qualifications:
- Bachelor's degree in Nursing or a related field
- [X] years of experience in case management or a related healthcare role
- Strong communication and interpersonal skills
- Ability to work collaboratively with a multidisciplinary team
- Excellent organizational and time management skills
- Knowledge of healthcare regulations and standards
- Proficiency in electronic medical records systems
